Dr. Alfred received his medical degree from Howard University College of Medicine in Washington, D.C., and completed residencies in psychiatry at Washington University in St. Louis and at Walter Reed Army Medical Center in Washington, D.C. Dr. Alfred served on active duty for over nine years with the Medical Corps of the United States Air Force, departing in 1971 with the rank of Lieutenant Colonel. His achievements were recognized with several military decorations, including the Legion of Merit. Dr. Alfred was the founding chairman and professor of psychiatry at Morehouse School of Medicine, Chief Psychiatrist at Morehouse Medical Associates, and Clinical Professor of Psychiatry at Emory University School of Medicine.

Dr. Alfred was appointed to a second five-year term on the Georgia Board of Human Resources and serves as a member of the Board's Executive Committee and Chairman of the Board's Committee on Mental Health, Mental Retardation, and Substance Abuse. He held several offices with the Georgia Psychiatric Association, including secretary and president. He was a Fellow of the American Psychiatric Association and a Solomon Carter Fuller Award recipient in 1994.
